If you live within our practice area (WV1 postal code) you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
It takes up to 5 working days for registration to go through.

Patients that move outside of East Park Medical Practice catchment area
All patients that move to a new address that is outside of East Park Medical Practice catchment area (WV1 postal code) must re-register with another GP surgery in their new catchment area.
Before patients are removed from the patient list Primary Care Support England will inform all patients in writing that they no longer live within the catchment area. This letter will also provide details on how to register with a new GP.
Patients will be given 28 days to enable them enough time to re-register with a new GP.
If required copies of hospital letters or copy of a patient medical summary can be provided to the patient.
Patients will also be provided with enough prescriptions to cover them until they are registered with a new GP practice in order not to compromise care.
